Verdict
«Brain Maker» (2015) caught a real scientific wave and then surfed it straight past the shore. The microbiome was the hottest topic in biology, and David Perlmutter, a board-certified neurologist, packaged it into a confident story: fix your gut bacteria, protect your brain from Alzheimer’s, autism, ADHD and depression. The problem is the confidence. A fact-check of the book found that roughly a third of its 103 sampled citations contradicted the claim they were attached to, and nearly half leaned on non-scientific sources for scientific points (Factual Wellness).
The core science — that gut and brain communicate — holds up well. The therapeutic promises do not. So this is a book whose good ideas you already get from the underlying research, and whose bad ideas can push a worried reader toward self-treatment. Our rating: «Read our notes instead». Same as our take on another overclaimed health bestseller, «Why We Sleep»: the topic is real, the salesmanship is the problem.
The big idea
Perlmutter’s thesis: the trillions of microbes in your gut are a hidden control panel for the brain. They shape inflammation, immunity and neurotransmitters, so what you feed them decides whether you stay sharp or slide toward disease. The prescription follows from the premise — feed the «good» bacteria with fiber, fermented foods and fat, starve them of sugar and gluten, and the brain benefits.
The first half is a fair reading of a young field. The second half treats correlation-heavy, mostly-animal science as settled clinical medicine. That gap is the whole story.
Key ideas
- The gut-brain axis is a two-way line. The gut and brain trade signals through the vagus nerve, immune messengers and microbial metabolites. This is now mainstream neuroscience, reviewed in depth by Cryan and Dinan and colleagues.
- Microbes make and modulate neuroactive chemicals. Gut bacteria influence serotonin precursors, short-chain fatty acids and inflammatory signaling. Influence is the honest word; Perlmutter often upgrades it to «control».
- Diversity looks protective. A richer, more varied microbiome tracks with better metabolic and immune markers. How to build it is where evidence and marketing part ways.
- Diet feeds the ecosystem. Fiber and fermented foods change the microbial community measurably. This is the book’s most defensible practical claim.
- Inflammation is a plausible bridge. Chronic low-grade inflammation is linked to mood and cognition, and the gut helps set inflammatory tone. «Linked» is not «causes and cures», though.
- Personal responsibility, oversold. The framing that your fork can override genetics and neurology is motivating and, for serious disease, misleading.
What holds up
The axis itself — confirmed. The microbiota-gut-brain axis is real and well-mapped. Cryan and Dinan’s landmark review and the later comprehensive account in Physiological Reviews lay out the vagus-nerve, immune and metabolite pathways. Perlmutter did not invent this, but he popularized a genuine field.
Fermented foods — confirmed, at the level of biomarkers. A Stanford randomized trial of 36 adults over 10 weeks found that a fermented-food diet increased microbiome diversity and lowered 19 inflammatory proteins, including interleukin-6; a high-fiber arm did not move those markers in the same short window (Wastyk et al., Cell, 2021; Stanford Medicine). This is the single result that most vindicates the book’s practical advice — and it measures diversity and inflammation, not curing dementia.
Diet quality matters for the brain — confirmed in the general sense. Diets heavy in whole foods, vegetables and healthy fats track with better long-term cognitive and cardiovascular health. That much is uncontroversial and connects to how you manage everyday energy and clear thinking.
What doesn’t
«Heal and protect your brain» — unsupported as stated. The subtitle promises therapy. No diet or probiotic has been shown in humans to reverse a neurodegenerative disease. The strongest test of the food-for-the-brain idea, the MIND-diet randomized trial, followed older adults for three years and found no significant difference in cognitive decline versus the control diet (NEJM, 2023). A brain-targeted diet, tested properly, did not slow decline.
Autism via the gut — mostly mouse-only, and contested. The exciting «microbes shape social behavior» studies are largely rodent work. A 2025 critique argued that human autism-microbiome studies are underpowered and that the causal claims outrun the data, concluding there is no evidence the microbiome causes autism (PubMed; coverage in The Scientist). Defenders point to early human trials, so grade this preliminary at best — not the settled cause-and-cure the book implies.
Depression and ADHD via diet — preliminary. The mood links are real as associations and promising in small studies, but they are nowhere near «change your breakfast, treat your depression». Treating them as interchangeable is the book’s recurring move.
The anti-grain, anti-gluten stance — unsupported for most people. This carries over from Perlmutter’s «Grain Brain». On release, dietitians and grain scientists called it misleading: the Mediterranean and DASH diets contain plenty of grains and track with lower dementia risk, and there is no evidence the 90%-plus of people without celiac disease or gluten sensitivity should avoid grains (FoodNavigator; The Health Sciences Academy).
The author’s track record invites caution. Perlmutter has drawn sharp criticism from scientists — epidemiologist David Katz called his work «way ahead of any justifiable conclusion», microbiologist Jonathan Eisen likened the pitch to a «snake-oil salesman», and «Grain Brain» was later found to contain plagiarized passages (Wikipedia summary of published criticism). He also markets branded supplements, which is worth knowing when a book’s advice conveniently points toward products.
Who should actually read it
Read the actual book if you study science communication and want a clean case study of how a real field gets oversold. Curious readers who can keep «interesting hypothesis» separate from «medical fact» may enjoy it.
Skip it if you are frightened about your memory, your child’s development, or a mood disorder, and are looking for what to do. The book’s certainty is exactly the wrong input for those decisions. One thing to try
The shift: stop treating food as brain medicine and start treating it as brain maintenance. Maintenance is a boring, durable policy: feed the microbiome variety, don’t chase reversal of anything.
First move: add one genuinely fermented food to your normal week — plain yogurt with live cultures, kefir, kimchi or sauerkraut — alongside the fiber you already eat. That is the intervention with the cleanest human evidence, and it asks nothing you have to buy from an author.

The gut-brain axis is one of the most exciting stories in modern biology, and Perlmutter told it two sizes too big. The connection is real; the cures are not. Eat more plants and a bit of fermented food because the evidence for microbiome diversity is decent, not because a book promised to protect your brain. When a health claim jumps from a mouse cage to a diagnosis, slow down.
